I certify that the information provided in this application is accurate and understand that any omission or
misrepresentation may result in the rejection of my application or my expulsion. I also agree that if there is
any fraud, embezzlement or theft, the last pay cheque will be retained by the employer for damages and
expenses incurred. It is agreed that if I am hired, I will be subject to a three (3) month probationary period,
during which time I may be laid off without any recourse. I hereby authorize Martial Excavation Inc. and the
Groupe Jalicika to verify the information I have provided in my application for employment and to proceed
with any verification.

referred to as "the company") to collect from any previous employer, company, institution, personal
information agent, establishment or individual, any information concerning me that they may hold and to
proceed with any relevant request, all for the following purposes verification of the information that I have
provided or should have provided in the context of my application for employment and, if I am hired,
maintenance and / or rectification of any information necessary for the performance of my employment
contract. For the above purposes, the company will act as my representative and all of the above persons
are hereby authorized to provide the company with the requested information.
This consent will remain valid for the duration of my employment. I acknowledge that I have carefully read
and understood the terms of this consent before accepting it, and that it is freely given.
